Congregate Housing

Is a shared living environment that affords each tenant the privacy of their own bedroom and bath, while promoting socialization through use of the common kitchen, dining, living area, and shower facility. An individual must be sixty years of age or disabled and meet income and program criteria.

There are four Congregate Housing sites located in the area. The sites are as follows:

50 Day Street, Fitchburg 18 Pond Street, Ayer 16 Ready Drive, Winchendon 116 Church Street, Gardner

Adult Family Care

Individuals, age sixteen years and older, are matched to a family caregiver who provides room, board, and daily personal care. Both the caregiver and the participant are assessed to determine the type of care that is to be provided. Personal interests are determined and become part of the care plan. The program social workers and nurses oversee the placements to ensure their success. The placement is afforded either with total private payment or partly with MASSHEALTH reimbursement.

The Supportive Housing Program

This new program is available to those residents of both the Gardner Housing Authority's Church Street development and the Fitchburg Housing Authority's Day Street complex. It offers Assisted-Living type services in a public housing setting. A Personal Care Homemaker is on-site twenty-fours a day, seven days a week. She is able to respond to after hour’s non-medical emergencies. A daily meal is provided. A Supportive Housing Coordinator is available at the site five days a week to assist residents with their problems or questions. People living there can be reminded to take their medicines. Special activity programs, trips, and speakers enrich the socialization of the residents.

Group Adult Foster Care

Provides daily personal care services to individuals 22 years of age and older, who are MASSHEALTH recipients and who are disabled and reside in public housing.
